Excuse Form

Excuses for Absence from School

If your child is sick or cannot attend school for an important reason, you must notify the school as soon as possible.

Here’s what to do:

• You must call the school by the second day of absence.
• Please call before 8:00 AM at:

Important:

• If the school does not receive a message, the absence will be marked as unexcused.
• The school may ask you to send a written excuse afterwards.
• If the student is under 18, parents or guardians must report the absence.
• From age 18, students must excuse themselves.
